The Pasadena Vital Records Office maintains records of births and deaths that occurred in the City of Pasadena. 

 

Vital Records hours are 8:00 am - 5:00 pm

Monday - Thursday (except holidays)

 

Our facility is located at 1845 North Fair Oaks Ave. just 1.5 miles North of the 210 Freeway.  Parking is available on Fair Oaks Avenue. The Vital Records Office is now located in Room 1610 First Floor.

 

 

Pursuant to California law only certain individuals are permitted to

receive an Authorized Certified Copy of a birth or death record.

What are the hours of the Vital Records Office?

 

The Vital Records Office is open Monday through Thursday, 8:00 am to 5:00 pm.  We are closed on Fridays, holidays and weekends.

How do I get to the Vital Records Office?

 

The Pasadena Public Health Department is located at 1845 N. Fair Oaks Avenue, on the corner of Tremont Avenue and Fair Oaks Avenue.  We are approximately 2 miles north of the 210 Freeway.

 

When you arrive, sign in at the reception desk at the Fair Oaks entrance

The Vitals Records office is located in Room 1610, near the reception desk.

 

 

Am I authorized to request a copy of a birth or death record?

 

You must be an authorized person in order to obtain a copy of a birth or death record.  This list includes the registrant, parents, grandparents, siblings, spouses or domestic partners.  Additionally, attorneys, law enforcement agencies, court ordered appointed persons and funeral directors may receive copies for official business.  Click here to find out if you are an authorized individual.

 

 

How much does it cost?

 

Birth records are $17.00 per certified copy.  

Death records are $12.00 per certified copy. 

 

For an additional $10.00, same day service is available for in-person requests if your application is received by 4:00 pm. 

 

For Express Service by mail, send a pre-paid mailer from an overnight carrier along with an additional $30.00 fee for each different birth or death record you request.

 

 

How do I obtain a copy of a birth or death record in person?

 

To obtain a birth or death record in person, you must complete an application that includes signing a sworn statement before the Vital Records Clerk (or person assisting you) and showing a valid ID, such as a driver’s license or passport.  The Birth Record Application or Death Record Application is available online or in our office. 

 

The cost for a birth record is $17.00 per certified copy, and the cost for a death record is $12.00 per certified copy.  Records are available for pick up the next business day if paid for before 4:00 pm, Monday through Thursday.  Your record will be mailed to you within 5 business days if you do not pick it up the next day.

 

We offer same day service for an additional $10.00 charge if your application is received by 4:00 pm, Monday through Thursday.  Your certified copy can then be picked up between 5:00 pm and 5:30 pm that same day.

 

 

How do I obtain a copy of a birth or death record by mail?

 

To obtain a birth or death record by mail, you must complete an application and sworn statement. 

 

The Birth Record Application or Death Record Application is available online.  The sworn statement must be signed in the presence of a Notary Public and mailed to the Vital Records office with your check or money order.  The cost for a birth record is $17.00 per certified copy, and the cost for a death record is $12.00 per certified copy.  Requests must include a self-addressed stamped envelope. 

 

Requests are processed within 5 business days from date of receipt.  Please note, if the sworn statement is not notarized, your request cannot be processed.

 

Express Service is available by mail for an additional fee.  You must include a pre-paid mailer from an overnight carrier along and an additional $30.00 for each different birth or death record you request.  Clearly note “Express Service” on the front of your mailing envelope, and your Express Service order will be processed the day it is received.

 

 

How long does it take to get a birth or death record?

 

In Person:  Your request must be submitted by 4:00 pm for pick up the next business day. 

 

Same day service is available for an additional $10.00 fee for each different birth or death record you request.  If you request same day service, your request must be submitted by 4:00 pm for pick up between 5:00 pm and 5:30 pm that same day.  If you do not pick up your certificate the next day, it will be mailed within 5 business days.

 

 

By Mail:  All mail requests are processed within 5 working days from date of receipt. 

 

Express Service is available for an additional $30.00 fee for each different birth or death record you request.  Express Service requests are processed upon receipt.  For overnight or express mail delivery, you must include a pre-paid mailer from an express mail or overnight carrier with your order.

 

 

What payment methods do you accept?

 

In Person:  Payment can be made in cash, money order, cashier’s check, Visa, MasterCard, debit or bank printed personal checks that include requestor’s street address.

 

By Mail:  Payment can be made by money order, cashier’s check or bank printed personal checks that include requestor’s street address.

 

Checks are payable to Pasadena Public Health Department.  A valid Driver’s License or ID number must be listed on your check.

 

 

Is there a charge for returned checks?

 

Yes, there is a $35.00 fee for each returned check.

 

 

What if the birth or death occurred outside the city of Pasadena?

 

The Pasadena Vital Records Office does not maintain records for births or deaths occurring outside the City of Pasadena.

 

Records for events that did not occur in the City of Pasadena but took place in Los Angeles County may be obtained from the Los Angeles County Registrar.  The Los Angeles County Registrar can be reached at (562) 462-2137 or visit their website at http://www.lavote.net/recorder/birth_death.htm.  For other California cities, call 411 and ask for the County Registrar for the city where the birth or death occurred.

 

 

How do I make changes to a birth record?

 

An amendment can be submitted directly to the State Office of Vital Records to correct spelling errors, add information to blank items, correct the spelling of parents’ names or correct statistical information (i.e., birth date error). 

 

Amendments CANNOT be used to completely change a first, middle or last name without a court order. More information can be obtained by going to the State website at www.dhs.ca.gov and selecting “Services” and then “Birth, Marriage and Death Certificate."  Forms for amending a birth record are available at our office.

 

 

How do I add the father’s name to a birth record?

 

Unmarried parents need to complete a Declaration of Paternity to add the father’s name to a birth record.  To obtain a Declaration of Paternity, you must go to the Vital Records Office in person, and our staff will assist you.  There is no charge for completing a Declaration of Paternity.

 

Married parents need to complete an Acknowledgement of Paternity to add the father’s name to a birth record.  To obtain an Acknowledgement of Paternity, you must go to the Vital Records Office in person, and our staff will assist you.  There is a $20.00 fee for this service.

 

 

What if I was born at St. Luke’s Hospital before 1950?

 

We do not have records for births that occurred at St. Luke’s Hospital before May 1, 1950.  To obtain copies of these records, contact the Los Angeles County Registrar at (562) 462-2137 or visit their website at http://www.lavote.net/RECORDER/BDM_Records.cfm.

 

 

What if I need an Apostille Seal (special seal certifying a document is a true copy of the original?

 

The California Secretary of State provides authentication of public official signatures on documents to be used outside the United States of America.  Once the signature on your document is confirmed, an Apostille is placed on the document verifying that the signature has been authenticated.

 

The California Secretary of State only authenticates records for use outside of the United States that are signed by a County Clerk/Recorder.  Records from the Pasadena Office of Vital Records CANNOT be authenticated.  You must purchase the birth or death record from the county where the birth or death occurred.  For more Apostille and/or authentication information, please visit the Office of State website at http://www.ss.ca.gov/business/notary/notary_authentication.htm.

 

 

What if I was adopted?

 

If you were adopted, your record is sealed and is available only from the Sacramento Office of Vital Records at (916) 875-5345.

 

 

How far back do the birth and death records go?

 

The Pasadena Office of Vital Records has records as far back as 1893 for birth records and 1897 for death records; however, these records must be requested from our archives.  There is no same day or next day service for any application for a birth or death record prior to 1930.
