Death Records


 

u  Death Certificates are $12.00 per certified copy.

 

u  Required information includes Full name of the deceased as it is listed on the Death Certificate.

 

Pursuant to California law only certain individuals are permitted

to receive an Authorized Certified Copy of a birth record.

Mail-In Requests


Send the completed Death Record Application and notarized statement your payment and a self-addressed stamped envelope to our office in Pasadena.

Please indicate on the application the number of copies you want. Requests are processed within 5 business days from date of receipt. Please note if the sworn statement is not notarized your request cannot be processed.

Express service is available by mail for an additional fee. You must send an additional $30.90 for each different death record you request. You MUST include a pre-paid mailer from an express mail or overnight carrier with your order. Clearly note “Express Service” on the front of your mailing envelope and your Express Service order will be processed the day it is received.

 

Certified mail service is available for an additional fee of $15.40.

 

For Express delivery outside of the continental U.S. please call (626) 744-6010 for information and additional charges.

 

Payment methods: Money order cashier’s check and bank-printed personal checks that include the requestor’s street address are accepted for payment. Checks are payable to Pasadena Public Health Department. A valid Driver’s License or ID number must be listed on your check.

 

 

Pursuant to California State Code 103650 the fee for any search of the files and records for a specific record when no certified copy is made shall be paid in advance by the applicant.  If a record is not located the fees for the death record will be retained as payment for a record search.

 

 


In Person Requests



To obtain a copy of a death record in person you must complete a Death Record Application and bring a valid form of identification such as a driver’s license or passport. You can get the application online or in our office.
 
 The Pasadena Vitals Records office is located at 1845 North Fair Oaks Avenue just 1.5 miles North of the 210 Freeway. Parking is available on Fair Oaks Avenue. The Vital Records Office is now located in Room 1610 First Floor. Business hours are 8:00 am - 5:00 pm Monday - Thursday (except holidays).

If you submit your request by 4:00 pm your record will be available for pickup the next business day. Records that are not picked up the next day will be mailed out within 5 business days.
 
Same day service is available for an additional $10.00 charge if your application is received by 4:00 pm Monday through Thursday. Your certified copy can then be picked up between 5:00 pm and 5:30 pm that same day.
